    Roaches urine smell neglect substantiated

    I would not recommend this facility. Leadership failed to communicate with nursing, the building is a shell of itself - roaches everywhere, a constant urine smell, and long-time staff who looked disgusted. My loved one had falls, an ER visit, and a delayed chair-alarm response; neglect was later substantiated. Look at other facilities and consider asking the state to intervene - they even served chocolate milk as a drink.
