    Nice facility but fatal neglect

I appreciated the timely, thorough intake, friendly knowledgeable staff, smooth admission, quick room assignment, good doctor coordination, wheelchair help, and the on-site salon, church, dining and holiday events - communication was generally reasonable. However my loved one developed a bedsore, later required hospice and passed away; I felt there was neglect and, despite a nice facility, I cannot recommend it.

    Summary review

Overall sentiment across the review summaries is mixed, with a substantial set of positive operational and social features reported alongside serious, isolated negative incidents that raise concerns about care consistency. Many reviewers highlight a smooth and responsive admissions experience, noting timely responses, a thorough initial assessment, quick room assignment, and assistance with mobility (wheelchair help). Communication and coordination with physicians are repeatedly praised, and staff are described as knowledgeable and friendly in several summaries. These operational strengths appear to create a family-friendly atmosphere that supports visitors and families; multiple mentions of holiday parties, invitations, an on-site salon and church, and cafeteria/dining options paint a picture of an environment that provides social and practical amenities beyond medical care.

Care quality presents a mixed picture. On the positive side, several reviewers express reasonable satisfaction and describe the facility as good or nice, implying an acceptable baseline of clinical and custodial care for many residents. Strong communication and coordinated doctor involvement are positive indicators for clinical oversight. However, the reviews also contain very serious negative reports: allegations of neglect, the development of a bed sore, hospice involvement, and at least one account mentioning a resident death and a recommendation against the facility. These are high-severity concerns that contrast sharply with the operational positives and suggest variability in the quality of direct care delivery. The presence of hospice involvement and a reported death in reviews does not by itself indicate misconduct, but when paired with reports of neglect and bed sores it signals that adverse outcomes have occurred for at least some residents.

Facilities and activities are consistently described as strengths. The availability of an on-site salon and church, regular dining services in a cafeteria setting, and social programming such as holiday parties are recurring positives. These offerings support resident quality of life and family engagement, and reviewers emphasize the family-friendly environment and invitations to events. The physical facility itself is described favorably in multiple summaries, contributing to overall satisfaction for some families.

Staffing and management impressions are mixed but spotlight communication and friendliness as notable strengths. Several summaries explicitly call out staff knowledgeability and friendliness, as well as strong communication and smooth administrative processes at admission. At the same time, the negative reviews point to potential lapses in direct resident care and oversight. The juxtaposition of administrative efficiency and reported neglect suggests inconsistency between front-office/coordination functions and bedside care/clinical follow-up for some residents. "Room for improvement" is a recurring, moderate-level critique that aligns with this pattern of generally adequate operations but uneven care outcomes.

Notable patterns and recommendations for anyone evaluating this facility: the dominant pattern is one of operational competence (admissions, coordination, amenities, communication) with isolated but severe negative incidents related to clinical care quality. The coexistence of praise for staff and amenities alongside allegations of neglect and serious adverse outcomes suggests variability in resident experience—some residents and families appear quite satisfied while others report serious problems. Prospective residents and families should weigh the facility's strengths in coordination, communication, and social programming against the reported incidents of neglect and wound care issues. It would be prudent to seek additional, up-to-date information such as recent inspection reports, staffing ratios, wound-care protocols, complaint history, and direct conversations with current residents or families to better understand how common or isolated the negative incidents are and whether management has taken corrective actions.

About Senior Rehabilitation & Skilled Nursing Center

Senior Rehabilitation & Skilled Nursing Center is in Nederland, Texas, with other sites in Port Arthur and Liberty, and it serves seniors in need of skilled nursing care and short-term rehabilitation after illness, injury, or surgery, and you'll find that the place offers both large private and semi-private rooms, including some with private bathrooms, kitchenettes, cable TV, phones, and Wi-Fi-all designed to make recovery and daily living a bit easier. The center admits residents for both long-term care and short rehabilitative stays, accepts Medicare, Medicaid, and Veterans benefits, and is government (county) owned, with 128 certified beds as of June 2025; the staff includes nurses on duty by physicians, available 24 hours a day, and support with medication, bathing, dressing, dining, and transfers is part of daily life for those who need it.

There's a full suite of therapies offered-physical, occupational, and speech therapy-plus specialized care for tracheostomy, wound, IV therapy, diabetes, and different kinds of cardiac care for those with conditions like CHF or COPD, and folks can get care after orthopedic surgeries too, like knee or hip replacements. Rehabilitation programs focus on building strength, improving mobility, and helping residents get as independent as their situation allows, and there are individualized programs that set goals and check progress along the way.

Meals come from a professional chef with restaurant-style dining, and special diets for allergies or diabetes are no problem, and residents can join in on the community's movie nights, arts and crafts, music groups, and all sorts of activities that happen every day, either indoors or outdoors since there are walking paths, gardens, a patio, and other outdoor spots for getting some fresh air or socializing. Some people like the fitness room or library, others prefer the spa or wellness room, and there's also a dedicated memory care unit for those who need extra support due to memory loss. Housekeeping, laundry, dry cleaning, and even move-in coordination are provided, so residents don't have to worry about those details.

The business side is run by an Owner, Director of Nursing, and Director of Business Development, and the community belongs to a network called Gulf Healthcare Centers, which means there's shared experience and support across different sites, including Port Arthur and Galveston. There's organized transportation and parking, too. Community events, resident-run programs, and group wellness activities keep people busy and give everyone a chance to connect. The facility is not a CCRC, it's focused on skilled nursing and senior rehabilitation rather than retirement living, and while it has a 1-star rating from the Centers for Medicare & Medicaid Services as of now, the community rating from residents or families is 8.4 out of 10. The building doesn't have Better Business Bureau accreditation, but the rating stands at A+. The care here covers a wide range, with skilled nursing, respite care for short-term stays, memory care, and all sorts of therapies aimed at keeping seniors as healthy and comfortable as possible in a home-like environment, and there's always staff around to help, day or night.
