    Skilled staff, unsafe for long-term

I had a mixed experience. The therapists, nurses and CNAs (Stephanie and Jay stood out) were skilled, caring and helped my loved one regain independence-clean, home-like facility that I'd recommend for short-term rehab. However administration and safety practices alarmed me: missed meds, unanswered call lights, denied showers, ant infestation, door-lock/safety malfunctions, poor communication and slow responses. I felt it was negligent and potentially unsafe, so I strongly warn against long-term placement.

    Summary review

Overall sentiment in the reviews is mixed, with many families and residents praising the facility’s cleanliness, updated environment, and particularly its therapy/rehab services, while a minority of reviews raise serious safety and care concerns. Several reviewers emphasize that Diversicare of Martin appears newer or recently refreshed (newer carpets, fresh paint) and is generally kept clean and tidy. Multiple accounts describe a warm, home-like atmosphere, neat dining areas, clean rooms, and weekly room checks. The activities program (including weekly church services) and an engaged activities director receive positive mention. Short-term/respite and rehab stays are repeatedly cited as strong points—therapists and rehabilitation staff are described as skilled, instrumental in recovery, and able to help residents regain independence and return home.

Staffing and direct-care perceptions are polarized. Many reviewers call the staff respectful, caring, professional, attentive, and knowledgeable; nurses and CNAs are praised for keeping families informed and for taking an extra-mile approach. Specific employees were singled out for exceptional care (Jay Wainscott for wound care and another staffer, Stephanie, for overall helpfulness), and at least one reviewer called it their favorite facility and the "best facility we could ask for." These positive accounts often relate to therapy successes, responsive communication from certain staff members, and a sense of trusted caregiving.

Conversely, a notable subset of reviews reports serious problems with direct care and management. Recurrent negative themes include missed medications, unanswered call lights, long response times, staffing shortages, and cases where showers were denied or residents were hurriedly dressed. There are multiple allegations of poor communication and unresponsive management, and at least one reviewer issued a strong warning about unsafe care practices and potential abuse risk. Other concrete safety/practical concerns include a reported ant infestation and a malfunctioning/locked door that created a safety risk or restricted exit. These issues point to inconsistent care quality and potential lapses in basic resident safety and infection control.

Dining and daily care also show a split picture. While the facility’s dining area is described as neat and some reviewers are satisfied with care routines, others report food delays, missed lunches, and improper diets. Rehabilitation and therapy are the most consistently positive element across reviews, with multiple accounts of skilled therapists helping residents recover after falls or hospital stays. Wound care receives a specific positive mention. However, the overall impression is inconsistent—some families reported excellent outcomes and timely communication, while others reported poor rehab care with severe consequences (one reviewer reported their mother’s rehab care was poor and that she later passed away).

Management and reliability are recurring points of concern. Several reviewers describe management as unresponsive or ineffective in addressing problems, creating distrust among families. Staffing variability (both shortages and individual differences in caregiver quality) is frequently cited, contributing to a wide range of experiences—from highly satisfied to strongly dissatisfied. The presence of both glowing and damning reviews suggests significant inconsistency in day-to-day operations or recent turnover impacting care standards.

Bottom line: Diversicare of Martin has clear strengths—clean, updated facilities; a welcoming, home-like environment; and notably strong therapy/rehab services with some staff members providing excellent, specialized care. However, there are serious and recurring complaints about medication errors, delayed responses to call lights, staffing shortages, poor communication, denied personal care, pest issues, and at least one safety-related door malfunction. These negative reports are severe enough that prospective residents and families should approach with caution. Recommended next steps for anyone considering this facility: tour the site in person, ask specific questions about medication administration and tracking, staffing ratios and turnover, call-light response times, infection/pest control practices, wound-care expertise, and how management handles complaints. Also ask to speak with the therapy team and request recent quality metrics (incident reports, staffing levels, and any corrective-action plans) to confirm whether the concerning issues reported in some reviews have been addressed.

    About Diversicare of Martin

Diversicare of Martin sits at 158 Mount Pelia Rd in Martin, Tennessee, and cares for people with a mix of health needs, since the place covers short-term rehabilitation, long-term care, skilled nursing, assisted living, memory care, and hospice care, so folks with complex medical issues or specialized needs, like those dealing with memory loss, can get what they need right there, and the staff includes English speakers along with people who know other languages, which helps folks feel understood, and since the care team handles both typical aging health problems and mental health or substance use challenges, you get a place that brings together a lot of knowledge under one roof. People who need therapy after an illness or surgery can go for short stays to work on recovery, and for anybody who can't go home safely, long-term care is right there, and some stay in the assisted living side where there's support for daily tasks but still a push for independence, since the staff tries hard to help people keep their dignity and relearn lost skills or pick up new ones for daily living. The whole goal is to lift spirits, give people hope, and encourage confidence, treating every resident as an individual with a personal story to be honored. CNA training happens onsite, which helps new caregivers learn what to do and keeps the staff freshly trained, and the facility keeps a comprehensive team for high-quality care, but right now they're not taking new patients. Amenities and features aim for comfort and make daily life a little easier or more pleasant, so people can feel welcome and supported every day, even though hours and some info aren't currently clear, and while there are connected services like Facebook access, the main thing is a safe and caring environment where medical needs, daily support, and personal stories matter.
