    Excellent care but safety concerns

I was very well cared for - the food was excellent, staff were friendly, attentive and helpful, my room and the facility were clean, activities kept me busy, and the rehab care was top-notch. However, I noticed poor phone service, encountered a few rude staff, and heard troubling reports of staff misconduct and safety concerns that management didn't always address. Overall I'd recommend this place for care and rehab, but visit, ask questions, and be aware of those reported issues.

    Summary review

Overall sentiment from the reviews is mixed but leans positive on frontline care, facility cleanliness, dining, activities and rehab outcomes, while including several serious negative allegations that raise safety and management concerns. Many reviewers described warm, attentive and knowledgeable staff who communicate well with families and provide high-quality hands-on care. Multiple comments emphasized cleanliness throughout the facility—including clean dining areas, rooms, and laundry—and noted a cozy atmosphere with comfortable common rooms, a patio or courtyard, and a private family area. Several reviewers strongly recommended the center, praised the meals, and reported good rehab outcomes, even describing "miraculous" recoveries after transfers from other facilities.

Care quality and staff behavior are recurring positive themes. Numerous summaries call staff "friendly," "attentive," and "helpful," and reviewers say staff took time to explain things, kept families informed, and delivered effective rehab. Some reviews explicitly call the staff "excellent" and "well-informed," and there are multiple endorsements that residents are well cared-for and happy. Activity programming also receives consistent praise: reviewers mention enjoyable activities, games, prizes, television, and that residents are kept busy and engaged. Dining is frequently described positively—terms used include "delicious meals" and residents expressing that they "love the food." Facility amenities highlighted include large rooms with attached bathrooms, clean shared spaces, a patio/outdoor view, and a private family area.

However, the review set also contains several serious and recurring negative allegations that cannot be overlooked. Multiple reviewers reported suspected or alleged staff misconduct, including drug use (specifically meth), affairs with clients' families, and theft from residents. There are also allegations of neglect and mistreatment, and at least one claim that the director ignored reports of misconduct. Another reviewer alleged falsified COVID documents. These are grave accusations that, if substantiated, would represent major threats to resident safety and trust. Related operational concerns appear as complaints about poor staff pay and no raises, rude behavior by some staff, poor phone service or responsiveness, and descriptions of some staff as unprofessional.

A notable pattern is the inconsistency of experiences: while many reviewers praise the center and highly recommend it, a smaller but vocal group reports deeply troubling incidents and recommends against the facility. This bifurcation suggests variability in care or in staff performance over time or across shifts/units. There are also comments about locked or restricted areas ("lockdown room" and closed doors in private areas) that some reviewers found concerning or notable; it is unclear from the summaries whether these are security measures or indications of problematic restrictions.

In summary, the dominant strengths reported are compassionate and communicative frontline staff, clean and comfortable physical spaces, good dining and activities programming, and effective rehabilitation services for many residents. The dominant risks reported are serious allegations of criminal or unethical behavior by some staff members and apparent management failures to address these claims. The reviews overall recommend a cautious approach: the many positive reports suggest the facility provides solid care and an engaging environment for many residents, but the severe nature of several allegations means prospective residents and families should seek verification—ask about staff screening and drug testing policies, incident reporting and resolution procedures, recent inspection and quality reports, staffing levels, and how management responds to complaints—before making a placement decision.

About Lonoke Nursing & Rehab Center

Lonoke Nursing & Rehab Center sits at 1501 Lincoln Street in Lonoke, Arkansas, and holds 77 certified beds for people who need inpatient or long-term care, and even though it doesn't have published details about its history or extra programs, it operates as a rest home and skilled nursing facility under the management of Ashley Thomley since December 2024, and it connects to groups like Catlett Care and Central Arkansas Nursing Centers. About 50 to 99 employees work there, including certified nurses, nursing assistants, physical therapists, and licensed medical staff, who all help people with round-the-clock supervision, medication help, personal care, and mobility needs. The nurse turnover rate is 48.2%, but there's a higher number of nurse hours per resident each day than the average, coming in at 4.38, which usually means more direct attention for each resident, and they still aim to help everyone get better, stay independent, and maybe return home when they're ready.

Residents have access to long-term and rehabilitation care, respite care for those recovering from illness or surgery, and special support for anyone with medical disabilities or needs for higher security-like protected areas that help keep folks safe if they're likely to wander. The facility also provides regular meals and snacks, though inspections have shown some problems with making sure food meets every person's dietary needs and stays warm and palatable, which rated as a level E deficiency, and the most recent inspection in July 2024 found 15 problems, while recent reviews marked 24 issues in total, including 2 related to infections.

There are amenities such as flat screen TVs, free Wi-Fi, activity programs, salon and barber services, and restaurant-style meals, all to keep folks comfortable and help their mental and physical health. Help with transportation is available, and you've got people nearby at all hours to help with daily activities and keep medication managed. The Administrator, Mr. Ashley Thomley, is the principal contact, and while there's no information posted about Better Business Bureau accreditation, the staff work together to give people in their care attention based on each person's unique needs, trying to maintain dignity and well-being for all residents.
