    Caring staff, inconsistent clinical care

I had a mixed but overall positive experience: compassionate, hardworking CNAs and nurses, top-notch daytime therapy, clean homey surroundings, great activities and family communication. However, I also saw basic care lapses - slow responses, bathroom and wound-care issues, rough night staff and inattentive management - so I wouldn't trust them blindly with a parent. If you choose this place, expect loving staff who go above and beyond but monitor care closely and hold management accountable.

    Summary review

Overall sentiment across the reviews is predominantly positive, with many reviewers praising Columbiana Health and Rehabilitation for compassionate caregiving, strong therapy and rehabilitation services, cleanliness, and a warm, family-like atmosphere. A consistent theme is that the majority of staff — CNAs, nurses, therapists, housekeeping, and kitchen personnel — are kind, attentive, and go beyond expectations to treat residents with dignity and respect. Several reviewers singled out leadership and specific employees (including the facility director and kitchen staff) for exemplary communication and personal attention, and many described the facility as a five-star, highly recommended option that fosters long-term relationships with residents.

Care quality and staffing: Therapy and rehabilitation are repeatedly cited as strengths: reviewers called the therapy staff top-notch and recovery-focused, and several noted successful rehab stays and good outcomes. Day-shift staff receive widespread positive comments for friendliness, efficiency, and responsiveness. CNAs and nurses are frequently described as compassionate and dedicated, contributing to an upbeat, homelike atmosphere where staff feel like extended family. Family communication is highlighted as a strong point — many families report excellent updates and supportive interactions with management and clinical leaders when issues arise.

Facility, dining, and activities: The facility is portrayed as clean, well-maintained, and homey, with praise for housekeeping teams who ‘‘go above and beyond.’’ Reviewers appreciate outdoor spaces, multiple room options, and a generally comfortable environment. Dining receives favorable mentions for good food, and activities such as bingo and pageants are valued for keeping residents engaged socially. While some noted that decor is dated, this did not generally detract from the overall positive impressions of the environment.

Management and communication: Most reviews commend management for being involved and communicative, with stories of leadership stepping in to correct issues and help residents improve. This responsiveness is an important reassurance for families. However, there is a meaningful minority of reviews that express distrust or dissatisfaction with management. These dissenting voices raise concerns about inattentive leadership or advise caution when entrusting the facility with vulnerable family members. Thus, perceptions of management vary — many families praise leadership, while a few report poor experiences that color their overall impression.

Notable concerns and variability: Despite the strong positive trend, a number of serious concerns recur in the negative reviews and should not be overlooked. Several reviewers reported problems tied to overnight/night shifts: slow responsiveness, basic care lapses, and in alarming cases emotional mistreatment and residents found in soiled or undressed conditions. Wound care problems were also reported by multiple reviewers, including claims of open wounds not being managed properly. Some families described the facility as not functioning like a true rehabilitation center at times, noting that therapy is unavailable on weekends and suggesting variability in the level and timing of therapeutic services. Instances of slow bathroom assistance and other basic-care gaps indicate inconsistency in performance across shifts and staff members.

Synthesis and recommendations: In summary, Columbiana Health and Rehabilitation receives many strong endorsements for its compassionate staff, effective therapy services, cleanliness, family-oriented atmosphere, and engaging programming. These strengths make it a highly recommended choice for many families seeking long-term care or rehabilitation. At the same time, several reviews highlight serious, intermittent lapses — particularly during night shifts and in wound care — and a minority express concerns about management responsiveness or trustworthiness. Prospective families should weigh the overall positive pattern but also ask specific, targeted questions during tours: inquire about overnight staffing and supervision, wound-care protocols and documentation, weekend therapy availability, response times for bathroom or toileting assistance, and how leadership handles reported incidents. Verifying these operational details, and speaking with current families and staff where possible, can help ensure the care level matches expectations and reduce the risk of encountering the negative issues described by some reviewers.

About Columbiana Health and Rehabilitation, LLC

Columbiana Health and Rehabilitation, LLC sits at 22969 Highway 25 in Columbiana, Alabama, and has 63 certified beds available for residents, and you'll usually find about 59 people living there each day, which keeps things personal and familiar, and it's run by a for-profit company connected to Northport Holding Operations, LLC, with NHS Management overseeing how everything's handled, and you'll see names like Leigh Cobb, Hilary Morris, Lynn Rasco, and Kristin Smith in charge at different times, so there's some steady hands guiding things, but also some changes now and then. The facility mainly focuses on long-term nursing home care, but it also provides skilled nursing for those who really need help with daily living, and now and again people use it for short-term rehab after a hospital stay before heading back home, so it's got flexibility for both longer stays and shorter recovery, but it's really designed so residents get attention that's more tailored to their unique needs, with nurses, aides, and therapists working together to keep people comfortable and safe.

There's complimentary Wi-Fi available in resident rooms, cable TV, and a restaurant-style dining area so meals feel a bit more social, and rooms also come with laundry and beautician services, which makes life a little easier, and when folks want some fresh air or just a quiet spot, they can spend time in a beautiful outdoor courtyard or one of the decorated indoor common areas, and these spots help foster a sense of community and encourage friendships. The staff put on daily activities to keep everyone engaged, and those social and recreational programs help break up the day and give residents something to look forward to, especially when nursing staff offers encouragement and support, though you should know the nurse turnover rate here is a bit higher than the state average, at 58.3% compared to 48.1%, so sometimes there are some new faces, but the average nursing hours per resident are a touch above the state average, landing at 4.04 hours a day, which can be reassuring.

Residents can use the facility's video tour if they want to see what it's like ahead of time, and visitors are welcome for in-person tours too, which helps families picture where their loved one would stay, and if needed, the facility has COVID-19 testing to help keep everyone safe. There's also a focus on giving information and support to seniors when they're making decisions about their future care and day-to-day needs. While most things run smoothly, it's fair to mention that there have been some care-related concerns, like a recent inspection report in April 2024 noting two deficiencies-one was related to infection control, and another involved pharmacy services and making sure residents are free from major medication errors, with some cases of actual harm reported but none at the immediate jeopardy level, and a small $8,018 fine followed a related infection violation.

Ownership is divided among names like James Estes and several family trusts, and the facility is committed to maintaining a home-like environment with professional, caring staff delivering assistance as residents need it. There's nothing fancy or over-the-top, but the goal is to make sure everyone feels cared for, safe, and has the social and physical support to live as well as possible.
