    Excellent rehab care, minor issues

I highly recommend this facility - I felt real peace of mind. The staff are caring, professional and rehab-focused: my loved one had excellent PT after open-heart surgery, daily room cleaning, great meals, plenty of activities/field trips, and private rooms in a very clean, well-maintained building. Downsides: decor and furnishings are dated and Medicaid limits some services; I'd also advise staying involved because there are isolated reports of slow call responses, medication mistakes and lost items. Overall, loving caregivers and top-notch recovery care.

    Summary review

Overall sentiment across the reviews is mixed but leans positive: a majority of reviewers praise the staff, cleanliness, and rehabilitation services at Collinsville Nursing Home Inc, while a smaller but significant minority report serious concerns about care consistency and specific incidents of neglect or mismanagement.

Care quality and clinical services: Many reviewers describe the nursing care and clinical services as "top-notch," highlighting strong physical therapy, effective rehab outcomes (including post-operative recovery like open-heart surgery), and a recovery-focused environment. Several notes emphasize excellent physical therapy, superior care quality, and therapists who help patients recover. At the same time, there are multiple, specific reports of clinical problems: mismanagement of medications (Haldol cited explicitly), insufficient rehabilitation in at least one account (~30 minutes/day), and incidents that required emergency care or resulted in UTIs. This pattern suggests that while the facility can and does deliver high-quality rehab and nursing care for many residents, there is variability in practice and at least occasional, serious lapses.

Staff behavior and culture: The dominant theme is that staff are caring, compassionate, and treat residents like family. Many reviewers say staff go above and beyond, have great bedside manner, and create a sense of safety and being loved. Activity and therapy staff are repeatedly praised. Conversely, other reviews describe utter disregard for patient well-being, inadequate dignity in care, and slow responses to call lights. The juxtaposition of glowing and severely critical comments suggests inconsistency across shifts, units, or individual caregivers: some staff and teams are described as exemplary while others are implicated in neglect or poor practice.

Facilities, cleanliness, and environment: Most reviewers report a clean, well-maintained facility; "very clean" appears frequently. Private rooms and daily room cleaning are cited positively. Some reviewers call the decor drab or the furnishings outdated, noting that the facility is "not fancy" and some rooms are not modern. There are isolated mentions of sanitation issues (sticky floors) and lost personal items. Overall, physical cleanliness and maintenance receive strong approval, while aesthetics and modernization are identified as areas for improvement.

Activities and dining: Activity offerings are consistently described as plentiful, with an active calendar and field trips mentioned. Reviewers appreciate the social and engagement opportunities. Dining receives at least one explicit compliment calling meals "excellent," though comments on dining are less frequent than those about care and staff.

Safety, incidents, and management concerns: Several reviews raise serious safety and management issues: slow call-light responses, medication errors, a patient left in a soiled diaper, lost clothing/personal items, and at least one case that led to an ER visit and a UTI. These are significant concerns because they point to systemic issues (staffing, supervision, medication management, or procedures for hygiene and infection control) rather than minor dissatisfaction. One reviewer explicitly cites "utter disregard for human life," and others say they would not recommend the facility. There is also a theme that Medicaid-related constraints may influence care options or resources.

Patterns and implications: The most salient pattern is substantial variability: many reviews depict a warm, clean, and effective rehabilitation-focused facility with caring staff and good outcomes, while a smaller subset report troubling incidents and systemic failures. This suggests that experiences at Collinsville Nursing Home Inc may depend heavily on timing, particular staff members, or specific units. For prospective residents and families, this variability argues for doing targeted due diligence: tour the facility at different times, observe therapy sessions, ask about medication management protocols, staffing ratios, call-light response times, and policies around personal belongings and hygiene. Clarify how the facility handles Medicaid residents and whether constraints affect care quality.

Bottom line: Collinsville Nursing Home Inc appears capable of delivering excellent, compassionate care—especially for rehabilitation—according to many reviewers. However, there are notable and serious negative reports that should not be ignored. The overall recommendation is cautiously positive but contingent: the facility has clear strengths in staffing, therapy, activities, and cleanliness, but families should investigate the potential for inconsistent care and ask direct questions about the specific concerns raised in reviews (medication handling, call response, hygiene protocols, and personal property safeguards) before committing to long-term placement.

    About Collinsville Nursing Home Inc

Collinsville Healthcare & Rehab is a large nursing facility located in Collinsville, Alabama, offering both short-term rehabilitation and long-term care services. With a capacity of 200 beds, it operates under for-profit, corporate ownership, and participates in both Medicare and Medicaid programs. While not part of a continuing care retirement community, this center provides comprehensive care for residents requiring various levels of medical and personal assistance.

Short-term rehabilitation services at Collinsville Healthcare & Rehab are designed to support patients who are recovering from hospital stays due to events such as strokes, heart attacks, infections, or accidental injuries. Approximately 51.9% of residents are able to return home after being discharged from short-term rehabilitation programs, highlighting the facility’s focus on recovery and transitional care. The staff strives to maintain a safe environment, with 0.5% of residents experiencing falls that result in major injuries and 5.2% of short-term rehabilitation stays leading to infections requiring hospitalization. During their stay, 14.7% of short-term residents have needed emergency room visits. Nurse staffing is an important aspect of care, averaging 4 hours and 8 minutes per resident per day to ensure that medical and daily living needs are consistently met.

Long-term care at Collinsville Healthcare & Rehab is structured to help residents maintain as much independence as possible while receiving daily assistance with essential tasks such as eating, dressing, bathing, and administering medications. A notable 87.7% of long-term residents have maintained the ability to move around, eat, and handle other common activities without assistance, reflecting the center’s commitment to supporting autonomy. The vast majority—99.3%—have received the annual flu vaccination, underlining an emphasis on preventive health measures. However, weekend nursing staff levels are reported to be lower compared with weekday staffing, which is an important consideration for families planning long-term care.

Health, fire, and safety at Collinsville Healthcare & Rehab are subject to routine inspections. The most recent state inspection reports indicate that the facility has effectively met key standards related to food preparation and other resident safety factors. Past inspection findings have identified areas for improvement such as enhancing communication of information to residents and healthcare providers during planned discharges and implementing robust infection prevention and control programs. These issues were found to present low potential for harm and affected only a few residents, and corrective actions were taken in a timely manner.

Collinsville Healthcare & Rehab offers a supportive environment for those recovering from acute medical events as well as individuals who need ongoing assistance with daily living activities. By providing a blend of skilled nursing care, dedicated staff attention, and a focus on both rehabilitation and long-term support, the facility aims to address the evolving needs of seniors and individuals with complex medical requirements.
