    Beautiful facility but inconsistent care

I found a beautiful, clean facility with engaging activities, strong therapy, and many caring, helpful staff - my grandmother loves it. However, I've also seen serious lapses: rude or hostile aides, rough handling, residents left soiled or on bedpans for hours, medication delays and inconsistent care. Mixed feelings - great staff and leadership in places, but insist on asking about staffing, night care, and safety before you commit.

    Summary review

Overall sentiment in the reviews for Nixa Nursing & Rehab is highly mixed, with a strong polarization between reviews that praise compassionate, skilled care and reviews that report serious care failures and safety concerns. Many reviewers emphasize exceptional aspects: named staff (RN Brittany and Melody, therapists Amanda and Kelly) are singled out for kindness and effectiveness, therapy services are repeatedly described as excellent, and several reviews highlight a clean, renovated, attractive facility with engaging activities, group exercise, and visiting physicians from multiple specialties. Multiple families described staff as friendly, professional, organized, and willing to communicate by phone, and some called out strong leadership and a trustworthy care team.

However, a significant and recurring set of negative reports raise serious concerns about resident safety and basic caregiving. Specific allegations include neglect (for example, a bedpan left for over four hours, delays in pain medication, and night-shift neglect), rough handling (painful turning, being dropped on the toilet), removal or tying of call buttons to chairs, and ignoring doctors' orders. Several reviewers described residents being left in soiled conditions for hours, being forced to feed themselves when unable, or having food served in bed while unable to sit upright. These are not isolated, one-off complaints in the dataset — multiple reviewers recount very similar incidents — and they point to systemic lapses in basic resident care and dignity for some shifts or some staff members.

There is a clear pattern of inconsistent care depending on staff or shift. Many families praised particular employees and the therapy department and described excellent day-shift experiences, while others reported hostile or neglectful behavior, especially at night or from specific nurse aides. This variability suggests uneven training, supervision, or culture across teams. Compliments about cleanliness, absence of odors, and a newly remodeled appearance are contradicted by other reports of a strong odor in a hallway near a resident's room and a non-working room A/C. Such contradictions further support that experiences can vary widely within the facility.

Dining and nutrition also show mixed feedback. Some reviewers found food “OK” while others reported poor dietitian/cooking quality and weight loss, prompting families to bring in food. Therapy and activities receive consistent positive mention — multiple reviews praise therapists, rehab outcomes, engaging activities, and exercise programs — which is an important strength for recovery-focused residents. Clinical oversight also appears in some positive reviews via visiting doctors from multiple specialties, which can be a benefit for medically complex residents.

Communication and administration are problematic areas cited by some families. Reports of poor mail delivery, a confusing name-use policy, and miscommunication with staff contribute to family frustration. COVID visitation restrictions were mentioned as limiting access (a common constraint in many facilities), but frustrations in communication and policy clarity extend beyond pandemic-related limitations. Several reviews highlighted great leadership and organized staff, but others felt management was not addressing or preventing the serious incidents they observed.

In summary, Nixa Nursing & Rehab appears to provide very good therapy services, has staff members who are compassionate and skilled, and offers a clean, updated facility with engaging activities. At the same time, there are multiple, specific, and concerning reports of neglect, rough handling, removal of call buttons, medication delays, and failures to follow medical orders. The most important pattern is inconsistency: many families report excellent care from specific staff and programs, while others experienced or witnessed serious breaches of care and dignity. For prospective residents and families, this means asking targeted questions before placement: inquire about staffing ratios and night-shift oversight, emergency call-button policies, protocols for incontinence care and pain management, staff training and handling procedures, how management investigates and resolves complaints, and whether specific praised staff or therapy services will be involved in care. If allegations of neglect or abuse are suspected or observed, they warrant immediate reporting to facility management and the appropriate long-term care ombudsman or licensing authority.

    About Nixa Nursing & Rehab

Nixa Nursing & Rehab sits at 1104 N Main Street in Nixa, Missouri, and provides nursing care and rehabilitation services for people who need either short-term help or long-term support, and the place is staffed with registered nurses and certified nursing assistants who work alongside on-call doctors who are around day and night, and these doctors come in often to check on residents or when needed, but there's also a range of therapists to help people recover, because the physical, speech, and occupational therapy here helps folks get back strength and function after illness or injury, and the therapy plans get made with each person's doctor to fit their needs, whether someone is working to speak clearly again, move safely, or handle daily tasks independently. Residents can choose either private or semiprivate rooms, and they don't have to worry about chores because housekeeping and laundry get done for them, while dietitians help plan menus to match health needs, and pharmaceutical services make sure medicine gets given properly and on time. People don't have to feel stuck inside, as wheelchair-accessible vans take residents to medical and dental appointments, and there's daily activities to join if they want some social time or just want to keep busy, while trained staff provide support for memory care, giving help to those living with cognitive challenges. Dentists, podiatrists, psychiatrists, and hospice caregivers also visit regularly, and the facility aims to meet all levels of care for comfort, health, and well-being, which means support is tailored for each person, whether that's more specialized care, promoting independence, or making sure they feel safe and at home, and even though the place can serve quite a few folks, with beds for up to 82 people and 62 Residential Care Facility beds, the focus stays on promoting both physical and mental health for everyone who lives there. CNA and CMT classes get offered for free, supporting staff growth, and you'll find transportation is always safe and reliable with their special vans that can handle wheelchairs, so residents can get where they need to go without worry.
