    Understaffed care led to neglect

Some staff were warm, welcoming, and ran nice activities, but my overall experience was worrying. Chronic understaffing, ignored call lights, delayed/incorrect meds, missed/poor medical oversight and communication (including COVID testing/notifications), and instances of neglect and unsafe care led to falls, infections and hospital readmission. Management was unhelpful, rooms and housekeeping were often poor, and family updates/transfer notices were inconsistent. I can't recommend this facility.

    Summary review

Overall sentiment across the reviews is highly mixed: many families and residents report excellent, compassionate care and effective rehabilitation, while a substantial number of accounts describe serious safety, staffing, and communication problems. Positive reviews highlight helpful and professional staff, engaging physical therapy that aided recovery and mobility, a generally clean and odor-free environment, responsive maintenance, and enjoyable meals and events. Several families explicitly credited the facility with strong end-of-life care, personalized attention, and meaningful improvement in their loved ones’ function. Administrative responsiveness and supportive office staff are also called out favorably in many responses.

However, an equally strong set of negative themes recurs in numerous reviews. Understaffing is a dominant concern: reviewers repeatedly describe long waits for assistance, unanswered call lights, and CNAs or nurses who are too busy (sometimes observed on phones or wearing earbuds) to provide timely care. That understaffing appears to drive many downstream problems—delayed medications, missed orders, slow or absent incontinence care, and inadequate monitoring of wounds and other conditions. Several reports describe delayed or improper wound care, infection risk, medication administration lapses, and at least one case where a patient was readmitted to the hospital and later died; these accounts indicate serious lapses in clinical oversight for some residents.

Staff behavior and consistency emerge as another major area of divergence. Multiple reviews praise specific nurses, CNAs, and therapists as warm, kind, and professional; families felt reassured and well informed by these staff. Conversely, other reviews recount rude or unprofessional conduct (including a reported problematic night supervisor), staff who blame residents for being sick, and incidents where residents were yelled at. The inconsistency is striking: the same facility is described as having both "wonderful, caring" teams and "neglectful, dishonest" staff, suggesting variability by shift, unit, or individual caregiver.

Facility operations and environment also show mixed signals. Many reviewers describe the building as clean, well-maintained, and without the stereotypical "nursing home smell," and maintenance is often lauded for responsiveness. Yet other reviewers report serious housekeeping lapses—beds stripped and not remade for hours, dirty bathrooms, full trash bags, and rooms that are uncomfortably warm because of no air conditioning. Food quality likewise varies: numerous reviews praise the dietary program and event meals as delicious, while a subset calls meals bland or even "disgusting." Activities, events, and special celebrations receive consistent praise from many residents and families.

Communication, documentation, and care coordination are frequent pain points. Positive reviews note timely updates from staff and a sense of good communication, but multiple complaints reference poor documentation, failure to follow physician or discharge orders (melatonin, constipation care, wound instructions), lack of notification about transfers or COVID exposures, and inadequate coordination with outside surgeons and appointments. Several families described difficulty locating loved ones’ personal belongings or being left out of important decisions. Discharge planning is repeatedly criticized as poorly executed in some cases, leading to frustration and risk.

Safety and clinical governance concerns are serious themes for many reviewers: medication mistakes or delays, improper Foley/bladder care, dehydration risk, unattended falls, and delays in starting indicated treatments (for example, Paxlovid) are specifically mentioned. Some families assert that management sided with staff instead of addressing complaints, and a few reviews characterize the facility as profit-driven in ways that hurt resident care. At the same time, other families explicitly trust the clinical care and report that staff were "on top of things" and communicated well about condition changes.

In summary, Heritage Hills Rehabilitation & Healthcare Center appears to provide exemplary care for many residents—particularly where there are strong individual caregivers, proactive therapists, and attentive maintenance and dietary teams—but it also shows systemic inconsistencies that create real risk for others. The dominant negative patterns involve understaffing, uneven clinical oversight, documentation and communication failures, and variability in housekeeping and food service. Prospective families or referral sources should weigh both sets of experiences: there are clear strengths in therapy, some nursing staff, maintenance, and activities, but there are also recurring safety and coordination concerns that warrant asking direct questions about staffing levels, night-shift coverage, medication administration protocols, wound-care procedures, air conditioning in rooms, and how the facility handles transfers, COVID communication, and discharge planning. The reviews suggest that outcomes at Heritage Hills may depend heavily on which team members are on duty and how well leadership enforces consistent standards of care.
