    Compassion turned to dangerous neglect

I found the place caring, very clean, and the therapy department and business office were impressive and genuinely helpful - aides were loving and some nurses went above and beyond, and staff supported us compassionately during my son's death. After new owners cut staffing, care declined rapidly: severely short-staffed, rude/unprofessional behavior, poor communication, medication errors (including an insulin overdose and a patient death), and alleged misconduct left me distrustful. Some residents clearly love it and the therapy/skilled nursing can be excellent, but approach with caution.

    Summary review

Overall sentiment across these summaries is mixed but leans toward concern due to several serious allegations despite numerous positive accounts of individual caregivers and therapy services. Multiple reviewers praise the rehabilitation and therapy department, noting strong speech and physical therapy programs and a friendly, informative director. Several family members and residents describe nurses, aides, and caregivers as caring, loving, helpful, and genuinely engaged with residents. The facility is also described as clean, and the business office has been called extremely helpful. Some reviewers specifically recommend the center for short-term skilled nursing or therapy stays and describe noticeable improvements after transfers in from other facilities.

However, a recurring and significant theme is a decline in care associated with changes in ownership and staffing. Several reviews state that new owners enacted staffing cuts that dramatically reduced the number of aides, leaving the facility short-staffed. Reviewers describe a rapid, "downhill" drop in quality of care following these cuts, with family members reporting that the level of hands-on attention and the number of aides declined markedly. These staffing concerns are tied in the reviews to concrete negative outcomes: medication errors, alleged negligent care, and in the most serious reports, an insulin overdose and a patient death. Those severe allegations are reported by reviewers and should be treated as reported claims; they indicate acute safety concerns that appear in multiple summaries.

Communication and management responsiveness are another frequent area of complaint. Several reviewers report poor communication, unanswered phone calls, lack of callbacks, and being unable to speak with their loved ones. Paperwork issues and references to an investigation were mentioned without follow-up from staff, leading to frustration. There are specific reports of staff yelling at a caller and denying families medical information, contributing to distrust. Conversely, other reviewers explicitly praise staff for going out of their way to help families and being top-notch caregivers — underlining a notable inconsistency in experiences depending on the specific staff on duty or timeframe of stay.

The reviews also contain allegations of staff misconduct or abuse, claims of dishonesty, and at least one account of a resident being dropped. These complaints, together with medication error reports, create recurring safety and trust concerns among reviewers. At the same time, multiple independent comments describe the environment as "wonderful," "caring," and "amazing," and some families express gratitude for the quality of care their loved ones received. This contrast suggests variability in the resident experience: some family/resident interactions are highly positive, while others are seriously problematic.

What is notably absent or infrequently mentioned in the provided summaries are details about dining, structured activities, or specific facility amenities — most comments concentrate on clinical care, staffing, and communication. The most consistent positive points are the therapy services and individual caregivers who are praised; the most consistent negatives are management decisions (staffing cuts), ensuing short-staffing, poor communication, and several reports of dangerous clinical mistakes and alleged misconduct.

In sum, the reviews present a facility with strong therapy capabilities and many compassionate staff members, but also with serious concerns raised by multiple reviewers about recent ownership/staffing changes, communication failures, and alleged clinical errors and misconduct. These patterns suggest that prospective residents and families should verify current staffing levels, medication administration protocols, incident reporting and investigation practices, and communication policies. If possible, an unannounced visit and direct conversations with multiple families of current residents, as well as review of recent inspection or complaint records, would help clarify whether the problems cited are isolated incidents or indicative of broader, ongoing issues.

About El Reno Post-Acute Rehabilitation Center

El Reno Post-Acute Rehabilitation Center sits on 66 beds and has served the local community for years, and when you walk in, you'll see right away that residents have access to assisted living, skilled nursing, memory care, and both short-term and long-term health services, so you'll meet folks there for just a bit after surgery as well as people who need ongoing help with everyday needs like bathing and dressing or getting around; they even offer respite care for families who need a short break. The Rehab to Home program gives people an option to recover before returning home, and because some folks need special help, the in-house therapy program offers physical, occupational, and speech therapy, working with people who've had strokes, surgery, joint replacements, diabetes, or brain injuries. You'll find a very full day if you want it, with community-sponsored and resident-run activities, daily scheduled programs, planned day trips, as well as educational sessions, and you'll see staff help plan everything to fit each resident's interests and health needs. Every room has air-conditioning, a private bathroom, kitchenette, and comes furnished, so it's comfortable whether you're there a little or a long while. The center has a beauty salon, fitness room, outdoor garden, wellness center, gaming area, small library, computer center, and a main dining room where staff serve three nutritious meals every day using restaurant-style service, offering options for those who need special diets, like for diabetes. Residents get nursing coverage for 12-16 hours with a 24-hour call system in place, and the team can help with medications, transfers, and other daily needs. The nursing staff can help manage pain, provide IV therapy, dialysis, support after surgery, and help with diabetic exercise and nutrition programs, and a doctor's services are there for both short-term and long-term care needs. If folks need to get to appointments or just special outings, the center arranges transportation, and if people want to join in, there are plenty of activities each week. For those needing extra support, especially after a stroke or injury, specialized rehabilitation programs aim to help each person get stronger based on what the patient needs most at the time. The focus is on personal support, care planning, and helping people keep their individuality while also meeting their medical needs, and all this comes in a setting that aims for comfort and respect, so whether you're staying short-term or this becomes home, people usually find the staff is known for their caring approach. This Medicare-certified facility is operated by Mary Ann Morse Healthcare Corp. and keeps its focus on older adults who need different types of care, with both medical staff and therapy teams working together to support residents' physical and emotional well-being.
