    clean facility but unsafe care

I had a mixed, often alarming experience. The facility is bright, very clean and newly renovated; many nurses, CNAs, therapists, front-desk and social-work staff were friendly, attentive and helped with rehab and discharge. But there were serious safety and trust issues - theft (belongings and even ashes missing), abuse/neglect, poor supervision, HIPAA/privacy breaches, unresponsive call bells and inconsistent/nighttime incompetence; my ventilator-dependent son was nearly harmed and had to be returned to the hospital. Because of those risks I cannot fully recommend it despite pockets of excellent care.

    Summary review

Overall sentiment across the reviews is highly polarized: a substantial number of reviewers praise Pathways Nursing and Rehabilitation Center for strong therapy outcomes, cleanliness, and compassionate staff, while a smaller but very vocal set of reviewers report serious safety, privacy, and care-quality concerns. The positive reviews frequently highlight effective rehabilitation (helping residents regain strength and return home), knowledgeable therapists and respiratory staff, attentive nurses and CNAs, and helpful social work and discharge planning. Many visitors and family members note that the facility is bright, clean, and newly renovated, which contributes to a comfortable setting for visits and recovery. Several reviews also single out professional front-desk staff and prompt issue resolution, stating the staff made them feel welcomed, supported, and well informed. External recognition (a U.S. News mention) and repeated recommendations by families reinforce the perception that Pathways can deliver good clinical outcomes and a reassuring family experience.

However, the reviews also include multiple and sometimes severe negative allegations spanning neglect, safety failures, theft, and unprofessional conduct. Specific concerns include unresponsive call bells, development of wounds, falls (including a fall from bed), and alleged inadequate care of high-acuity patients such as those on ventilators. There are several grave accusations describing near-fatal outcomes, infrequent physician visits, and cases where residents were returned to the hospital after being inadequately tended. Theft and missing personal items — including an allegation that ashes were stolen — alongside reports that family-provided supplies were used on other patients, point to lapses in supervision and property safeguards. Privacy concerns and HIPAA violations are also reported, including persistent automated texts and calls to families and alleged breaches of confidentiality.

Staffing and management emerge as a clear pattern underlying many of the mixed experiences. Positive reviews tend to describe caring, attentive day staff, effective therapy teams, and responsive administration. Negative reviews, by contrast, repeatedly cite understaffing, insufficiently trained personnel, incompetent night supervisors, and management failures. This suggests inconsistent staffing levels or variable staff competency across shifts and units. Several reviewers explicitly call for investigations, and some insist the facility should be shut down. The presence of a previous name (Northwoods) in negative context suggests there may be historical quality or reputation issues that still influence family perceptions.

Facility condition and environment are among the more uniformly positive themes: multiple reviewers mention a clean, bright, and renovated facility with comfortable spaces for families. However, the positive physical environment is contrasted by reports that the facility can feel 'hospital-like' to some visitors — an interpretation that may stem from clinical acuity of residents or institutional atmosphere rather than cleanliness. There is little commenting on dining or activities in these summaries; neither strong praise nor criticism of food or programming appears prominent in the provided reviews, so those areas remain unclear based on available feedback.

Communication and family experience are mixed. Several families appreciate regular updates, smooth transitions, and social work support that helped with discharge and community resources. Others describe poor reception (no greeting), persistent automated messages, privacy breaches, and unprofessional communication. The combination of highly attentive staff reports and troubling accounts of dishonesty or rudeness suggests the family experience can vary widely depending on timing, individual staff members, and perhaps specific units within the facility.

In summary, the reviews portray Pathways Nursing and Rehabilitation Center as a facility capable of delivering excellent rehab care, compassionate staff interactions, and a clean, family-friendly environment for many residents. At the same time, there are multiple, serious allegations — including neglect, safety incidents, theft, and privacy violations — that warrant caution. The overall pattern is one of variability: many positive experiences co-exist with a smaller number of acute negative incidents. Prospective residents and families should weigh both sides, ask targeted questions about staffing ratios (especially night shift coverage), incident reporting and resolution procedures, property safeguards, privacy protections, and supervision for high-acuity patients. Where possible, verify recent inspection reports and speak with current families or ombudsmen to clarify whether the serious issues reported have been addressed and whether there is consistent, reliable care across all shifts and units.

About Pathways Nursing and Rehabilitation Center

Pathways Nursing and Rehabilitation Center sits on Providence Ave. in Schenectady, NY, and it's a place that offers a lot of nursing and therapy services, whether someone needs short-term rehab or long-term care, and the building itself has these family-friendly dining spaces where folks can sit down together, chat, or spend time with loved ones while eating, which helps people feel more at home and encourages them to be a bit more independent, and the food there comes from a professional staff that pays attention to each person's nutrition, which might be one reason why people seem to enjoy their meals, and they even fixed up their dining area not too long ago, making it a spot that feels inviting for both socializing and relaxing, which can matter a lot during tough times.

They have a big team, with nurses, occupational therapists, physical therapists, respiratory therapists, and a long list of doctors, including those in pulmonology, physiatry, wound care, psychology and psychiatry, podiatry, pediatrics, and dentistry, so people coming in with different health needs can usually find the right care, and they also run a lot of programs, like recreation activities led by a Recreation Director, and those programs aren't a one-size-fits-all thing, since they're set up so residents can do what's enjoyable to them or helps them heal, and there's a special focus on both adult and pediatric brain injury rehab, even going as far as to have the Stepping Stones at Hilltop program for children from birth to age nineteen, and there's trauma brain injury care and certified brain injury care for adults as well, plus things like respiratory therapy and ventilator care, with specific services for tracheotomy needs, so people don't have to look elsewhere if their care is complicated.

They've got twenty-four-hour nursing, hospice, respite care, behavioral management, IV therapy, subacute rehab, and complex medical support, and there are also services for peritoneal dialysis in Cortland if folks need dialysis close to home, and staff work hard on things like discharge planning and family guidance, making sure new residents or people coming from a hospital have tours and help from the Admissions Coordinator, since starting at a new place can be stressful, and the staff-who can work part-time, full-time, or per diem-seem both experienced and caring, and people looking for pediatric services will find them too, which you don't see in every facility like this. The recreation staff plan things for everyone, and the building has different amenities to make recovery or longer stays more comfortable, with Meta AI and "Meta" features marked for more specialized support.

The facility accepts people looking for inpatient care, rehabilitation, neuropsychological testing, ventilator services, and school programs connected to therapy, plus they offer family support and education all under one roof, and the place puts a lot of focus on helping people stay as independent as possible, even for those who need a lot of medical care, and they do have an in-house pharmacy. Pathways Nursing and Rehabilitation Center is rated 4.1 based on 40 reviews, which tells you that some people are pretty happy with the care, and it stays busy serving Schenectady, Niskayuna, and the surrounding area for those who need skilled nursing, longer stays, or rehabilitation after an injury or hospital visit.
