Jamaica Hospital Medical Center - TCU provides top-quality care to patients who are transitioning from an acute hospital stay to a safe return home. The Transitional Care Unit offers a nurturing environment where patients can receive multi-disciplinary services and continued care as they recover from their medical issues or undergo sub-acute rehabilitation management. The goal of the TCU is to help patients reach their maximum level of functioning and reduce the chances of unnecessary hospital readmissions.
The TCU team at Jamaica Hospital Medical Center works closely with patients and their families to ensure that their needs are met even after they are discharged from the acute hospital setting. The unit consists of a seven-bed facility within the hospital, where patients can receive individualized care tailored to their specific needs. Patients who may benefit the most from the TCU services are those recovering from conditions such as stroke, heart attack, surgery, or other medical issues that require additional recovery time.
To be admitted to the Transitional Care Unit, patients must meet specific criteria and require daily medical, skilled nursing, and/or rehabilitation services. Patients in critical condition are not eligible for treatment in the TCU. The TCU at Jamaica Hospital Medical Center is staffed by a team of Board Certified physicians, nurses, and allied health professionals who use a multidisciplinary approach to meet the needs of their patients. The TCU is part of a project developed by the Department of Health and is one of only a few designated units in New York State offering these specialized services.
