    Short-staffed, unsafe care; moved out

I had a mixed but mostly negative experience. The building looks nice from the outside, and some therapists and aides were fantastic - they helped my loved one regain independence after hip rehab - but overall care was inconsistent and unsafe. The place seemed short-staffed and unresponsive (we waited hours for help, call lights ignored), with medication delays/overmedicating, poor monitoring, multiple falls and infections, and even reports of a death after a fall. Some staff were kind and social activities/food were fine, but too many nurses and admin felt uncaring, untrained, or distracted. Because of the safety and neglect issues I moved my loved one and cannot recommend this facility.

    Summary review

Overall impression: The reviews for Ambassador Health of Lincoln are highly mixed, with a clear split between strong praise for individual clinicians and therapy services and recurring, serious concerns about nursing care, safety, and management. Many reviewers recount excellent rehabilitation outcomes, compassionate individual caregivers, and a pleasant facility appearance. However, an almost equal number of reviews describe dangerous lapses in basic nursing care, medication management, and responsiveness that have led to infections, hospital readmissions, falls, and even death. This polarization suggests inconsistent performance across staff and units and a facility where positive outcomes are possible but not reliably delivered.

Care quality and clinical concerns: One of the most frequent and serious themes is unsafe or inconsistent clinical care. Multiple reviews cite medication errors (including delayed meds, missed medications, overdoses), overmedication, and oxygen mismanagement that resulted in hospital readmission. Reviewers also reported poor pain control, delayed responses to clinical needs, and insufficient monitoring of high fall–risk patients; several described multiple falls and at least one death following a fall. Infection issues (pneumonia and urinary tract infections) and subsequent hospital transfers were also mentioned. Together these items point to systemic problems in clinical oversight, medication administration processes, and patient safety practices.

Staffing, responsiveness, and caregiving: Understaffing and slow responsiveness recur across many reviews. Specific anecdotes include extremely long waits for assistance (one report of five hours), patients left on bedpans or with call lights ignored, lack of bathing, and general neglect. At the same time, many reviewers single out specific staff — notably rehab therapists, aides, and some nurses — as caring, skilled, and instrumental in recovery. This creates a consistent pattern: pockets of highly competent, compassionate personnel provide excellent care, but staffing shortages, poor training, or low morale lead to other areas of neglect where residents do not receive timely or adequate basic care.

Rehabilitation and therapy: Rehab services receive the most consistent praise. Multiple reviewers reported successful outcomes after hip or other surgeries, regaining independence due to strong physical and occupational therapy, and high-quality rehab equipment. Some reviewers believe the facility is driven by Medicare/rehab revenue, which aligns with reports that therapy units often appear more focused and better staffed than long-term care units. Nonetheless, there are also reports that therapy was limited or restricted in some instances, suggesting variability in how rehab is delivered depending on the unit or individual circumstances.

Facilities, dining, and atmosphere: The physical appearance of the facility is described variably. Several reviews call the building beautiful, well-kept, and comparable to a high-end hotel or country club. Others describe a gloomy atmosphere, state of disrepair, or filthy rooms. Dining opinions are mixed but lean positive in some accounts: some residents enjoyed meals and found food better than expected, while other reviews complain of cold meals delivered to rooms. Social engagement and activities were praised by some (bird-watching, family-like social interactions), while other families said promised activities did not occur and residents were not encouraged to walk or leave rooms.

Management, communication, and culture: Reviews indicate management and cultural issues that may contribute to inconsistent care. Complaints include dishonesty during care meetings, bullying (reported in the kitchen), low staff morale, and a perception that the facility prioritizes payment source/rehab admissions over individualized long-term care goals. Some reviewers noted that Medicaid is not accepted or that financial limitations affected placement, which is an important practical constraint for families to consider. There are also accounts of staff appearing unmotivated or condescending, contrasting sharply with other reports of warm, family-like interactions.

Recommendations and patterns to consider: Prospective residents and families should approach Ambassador Health of Lincoln with caution and perform targeted due diligence. Key areas to probe during a tour or intake include medication administration and safety protocols, staff-to-resident ratios on the specific unit of interest, fall-prevention measures, infection control practices, and how the facility handles clinical incidents and family communication. Ask specifically about therapist-to-patient ratios and whether the positive rehab experiences reflected in some reviews apply to the unit you would use. Given the divergence of experiences, try to meet or observe the specific staff who will be caring for your loved one and request recent state inspection reports and incident records.

Bottom line: The facility demonstrates the capacity for excellent rehabilitation and compassionate care delivered by individual staff members, but multiple, serious and recurring complaints about medication management, neglect, responsiveness, safety, and management practices create substantial risk. Families who value high-quality, consistent nursing supervision and safety should verify current staffing, safety protocols, and unit-specific performance before placing a loved one at Ambassador Health of Lincoln. Conversely, those primarily seeking strong, outcome-driven rehab may encounter positive experiences but should still confirm consistency and oversight on the particular rehab unit.

    About Ambassador Health of Lincoln

Ambassador Health of Lincoln is a for-profit health care provider that's been family-owned and locally operated for over forty years, and they care for an average of about 72 residents a day within a facility that has 122 certified beds, so you might notice it's a busy place but not too crowded. The facility gives 24-hour skilled nursing care and helps with recovery after a hospital stay, focusing on both short-term rehab and extended care for people who need to stay longer. People can find both private suites and semi-private rooms, and the set-up supports safe transitions home, which is helpful when getting back on your feet. Ambassador Health of Lincoln provides adult pulmonary and ventilator specialty care, and there's an entire pulmonary unit with in-wall oxygen and its own entrance, which isn't something you see everywhere. There's also on-site hemodialysis for those who need it and special care for winding and healing wounds, including Wound V.A.C. therapy. The team manages tracheostomy and enteral feeding, offers IV therapy, and provides care for people with traumatic brain injuries, so they handle a variety of complex needs. Their rehabilitation services use things like the AlterG Anti-Gravity Rehabilitation Treadmill, and therapy goes beyond just the basics, including physical, occupational, and speech therapy, as well as nutritional counseling, all through Spectrum Rehabilitation. People here get access to certified dietary and pharmacy services, too, and there's advanced respiratory therapy on site. Extended care covers things like post-acute cardiac rehab, help after orthopedic injuries, and ventilator weaning. Assisted living and independent living options are also available, along with pediatric specialty care, so it's not only for seniors. The community is Medicare and Medicaid certified. The place says it supports physical, mental, emotional, and spiritual wellness, and the aim is to create a person-centered environment where people feel respected while they're healing or living long-term.
