Originally founded as a Poor Farm, the facility has since evolved into a state-of-the-art, 190-bed building located near Northern Illinois University.

Certified to participate in Medicare and Medicaid programs, DeKalb County Rehab & Nursing Center provides a secure environment for residents, including a specialized Dementia Care Unit for those with Alzheimer's disease and Related Dementia.
Admissions are open to individuals regardless of race, color, national or ethnic origin, age, religion, disability, sexual orientation, gender identity, or veteran status.
