    Inconsistent care dangerous neglect unreliable

I'm torn - a few nurses and aides were wonderful, attentive and the facility can be clean and organized. But care was wildly inconsistent: poor communication with families, unanswered nurse call bells/after-hours unavailability, therapy and rehab staff often rude, and orders frequently not carried out. I saw dangerous neglect - missed baths, sheets unchanged, lost items/brace, wrong meds, oxygen not hooked up, and rehospitalizations. Because of repeated safety and communication failures I would not trust this place for long-term care despite some compassionate staff.

    Summary review

Overall sentiment across the review summaries is highly mixed, ranging from strong praise for specific staff members and therapy services to serious and repeated allegations of neglect, unsafe care, and poor communication. Several reviewers described positive, even excellent, experiences centered on effective physical therapy, caring CNAs, and a clean, organized environment that eased transitions from hospital to rehab. These positive reports emphasize attentive staff who know residents by name and, in some cases, high-quality food and an accommodating, rehab-focused approach.

However, an equal or larger number of reviews report troubling patterns. The most consistent negative themes are poor communication with families and caregivers, nursing staff unresponsiveness (including unanswered call bells), and variability in the quality of care depending on shift or individual staff member. Multiple accounts describe basic personal care failures — residents not bathed or showered, sheets not changed, and assistance with toileting or bathroom needs not provided as ordered. Some reviews assert that patients were left on the floor or unattended for long periods, and at least one family reported an amputee with dementia being neglected. These reports point to potentially serious lapses in day-to-day caregiving.

Safety and clinical concerns appear repeatedly: medication errors, critical orders not carried out, oxygen not hooked up for sleep, and wounds treated without informing caregivers. There are allegations of lost braces and stolen clothing at discharge, and at least one report of a discharge that families considered unsafe. Several reviewers attributed hospital readmissions to inadequate care while at the facility. Short-staffing and insufficient qualified nursing assistants are mentioned as likely contributors to these lapses. Reviewers also describe administrative shortcomings: poor follow-up, unreturned phone calls, after-hours unavailability, and slow or ineffective discharge planning and patient teaching.

Facility issues were reported inconsistently but seriously in some cases — extended periods without hot water and reports of pest evidence (rat droppings) and foul odors. Cleanliness is a polarizing topic: while some reviews describe the facility as clean and organized, others report dirty conditions and a negative atmosphere. Dining received mixed feedback; some reviewers praised the food as excellent, while others called it horrid. Therapy services likewise vary in quality and frequency — some residents received excellent PT, while others reported therapy occurring only rarely.

Staff behavior and culture show wide variability. Several reviews single out wonderful, compassionate CNAs and nurses who provided top-notch care, while others describe staff as rude, disrespectful, or even abusive. There are specific mentions of poor attitudes from physical therapy and rehab staff in some cases, contrasted with high praise for PT from other families. This inconsistency suggests uneven training, supervision, or staffing stability. Families repeatedly note frustration with after-hours coverage and an inability to reach administrators or clinical staff when issues arise.

Taken together, these reviews indicate that Apple Rehab Coccomo can deliver very good short-term, rehab-focused care under the right circumstances — particularly when experienced, attentive staff are on duty — but it also has recurring, serious weaknesses that have resulted in neglect, safety incidents, and major family dissatisfaction. The patterns most frequently cited are communication failures, staffing shortages, inconsistent nursing care, medication and discharge errors, and occasional facility maintenance/cleanliness problems. Prospective families should weigh the positive reports of strong PT and caring aides against the documented risks, verify current staffing and supervision levels, and closely monitor care, especially for vulnerable residents or long-term placements.

    About Apple Rehab Coccomo

Apple Rehab Coccomo sits in Meriden, Connecticut, and has 100 certified beds, usually serving around 91 residents each day, and folks will notice that the place gives private rooms with in-room refrigerators, 32-inch flat screen TVs, private showers and baths, electric beds, phones, wireless internet, and renovated spaces that try to feel like home, and there's an on-site beauty salon along with laundry, maintenance, and housekeeping services to help residents stay comfortable during their stay. The facility runs 24 hours a day with staff offering skilled nursing care, dementia care, hospice care, respite care, short-term rehab, long-term care, and sub-acute care, and has a big rehabilitation gym with the latest therapy equipment and electric beds, along with physical, occupational, and speech therapies for both short-term and longer stays, as well as outpatient treatments. Some unique offerings include specialized programs like Dysphagia Therapy and Lymphadema Certification, speech language pathology with swallowing checks and memory retention plans, occupational therapy covering splinting, everyday skills training, eating support, and even cooking practice, and physical therapy programs helping with mobility, balance, bracing, and conditioning, and Apple Specialty Services such as home safety reviews, family and resident education, and discharge planning. There are recreational activities, regular parties, picnics, chapel and religious events, and support groups aiming to give social support for everyone, and the dining tries to offer a nicer experience for the residents. The facility is a private company, part of the Apple Rehab group, and has been around since 1976, employing around 51 to 200 people, and families will want to keep in mind that recent health inspections have found some issues including incomplete care plans, problems with upholding a homelike and safe environment, cleanliness and infection control concerns, 25 total deficiencies in a recent report, and a nurse turnover rate higher than the state average with fewer nursing hours per resident per day than is typical for Connecticut. Apple Rehab Coccomo stays open every day of the week, and the facility keeps a hospital-type setting with some hotel-style comforts, making it a place for both recovery and longer term support, but it's always good for families to review recent inspection reports to see how the facility is doing right now.
