    Excellent therapy, inconsistent nursing care

I had a mixed stay. The rehab team and therapists were excellent - great OT/PT workouts, engaged staff, and sunny, clean rooms that felt welcoming. However nursing and bedside care were inconsistent: slow call responses, rude or inattentive shifts at times, medication and hygiene lapses, and poor communication about medical status. I'd recommend this facility for short-term rehab if you monitor meds and nursing closely - I'm grateful for the helpful staff who made my recovery possible.

    Summary review

Overall sentiment: Reviews for The Californian - Pasadena are mixed but lean toward positive for short-term rehabilitation stays, with multiple reviewers praising the facility for excellent therapy, compassionate staff, and a clean, home-like environment. Many families reported marked improvement during stays, highlighting outstanding physical and occupational therapy, attentive nursing and caregiver staff, and an atmosphere conducive to recovery. Several reviewers explicitly called the facility "the best" or a "must-visit" for rehab and short-term care, and multiple individual staff members and therapists were praised by name.

Care quality and staff: A recurring strong theme is the dedication and warmth of the caregiving and therapy teams. Reviewers frequently described nurses, CNAs, therapists, and front-desk personnel as welcoming, professional, and genuinely concerned for residents' wellbeing. Physical and occupational therapists received particular acclaim for effective, motivating programs that supported independence and recovery. The facility's administrator was also complimented for being family-oriented and caring. However, this positive view is tempered by numerous reports of inconsistent staff performance. Several reviews describe slow call-light response times, missed pain or medication doses, and occasional rude or unprofessional behavior from specific staff members. A few reviewers went so far as to allege abusive conduct or neglect; these reports are serious but appear alongside many positive accounts, indicating variability across shifts or personnel.

Safety, medications, and clinical concerns: Medication management emerges as a notable concern in multiple reviews. While some families reported timely medication administration, others reported missed doses or medication errors. There are also complaints about poor communication with medical staff—specifically a lack of clear doctor contact and unclear medical status updates. Reports of inadequate follow-up, weight loss, and decreased appetite raise questions about monitoring and nutritional support for some patients. A minority of reviewers explicitly mentioned infection control issues and poor hygiene or incontinent care (diapers not changed, diarrhea incidents), which are serious and recurrent enough to warrant attention.

Facilities, cleanliness, and environment: Many reviewers praised the facility's cleanliness, bright and tidy rooms, and a welcoming foyer with an easy-to-use check-in kiosk. The environment is frequently described as "home-like," peaceful, and conducive to healing. Housekeeping is commended by several families for keeping hallways and rooms clean. That said, isolated reports contradict this, describing unmade beds, soiled bedding, or untidy diaper changes. Some reviewers noted the facility feels dated in places and mentioned occasional problematic roommate situations early in a stay.

Dining and daily living: Opinions on food are mixed. Several reviewers praised meals as well-balanced and accommodating of dietary restrictions, with some calling the food "terrific." Conversely, other reviewers reported cold trays, minimal eating by patients, and dissatisfaction with dietary quality. Activity programming and staff efforts to engage residents were highlighted positively in a number of summaries, with an activity calendar that inspires participation mentioned specifically.

Management, communication, and billing: Positive comments cite a caring administrator and responsive front-desk staff, with an efficient check-in process that gives families a sense of security. Negative comments focus on poor communication from clinical leaders and occasional unexplained charges, which left at least one family feeling surprised and unsettled. The pattern suggests management may be effective at creating a welcoming atmosphere and supporting therapy-driven outcomes but needs to address clinical communication and billing transparency.

Patterns and overall impression: The strongest, most consistent positives center on rehabilitation (PT/OT), friendly and professional staff members, cleanliness in many areas, and an overall restorative environment for short-term stays. The clearest negatives relate to inconsistent nursing responsiveness, medication and hygiene lapses, and troubling reports of abuse or neglect in isolated instances. Taken together, the reviews portray a facility that can provide excellent short-term rehab and recovery care for many patients, with standout therapy and compassionate staff, but with variability in day-to-day nursing reliability and some serious quality-of-care lapses reported by other families. Prospective residents and families should weigh the frequent positive rehabilitation outcomes and warm atmosphere against the documented concerns about medication management, call response times, hygiene, and communication. If considering this facility, families may want to confirm medication protocols, staff-to-patient ratios, communication processes, and billing practices up front, and monitor care closely during the first days of a stay to address any roommate or hygiene issues promptly.

    About The Californian - Pasadena

The Californian - Pasadena is a skilled nursing facility with 82 certified beds and usually around 57 residents each day, and while Brandon Bales and Robert Siew have managed it since April 2024, with ownership interests noted for Abraham Bak and Menachem Gastwirth, the place does have a record that shows both good and bad, for example having 49 total documented deficiencies over recent inspections, including one for not planning a resident's discharge to match their needs and goals, which poses a potential for more than minimal harm, another for not keeping areas free from accident risks or ensuring enough supervision, which caused actual harm but not immediate danger, and five infection-related deficiencies that don't meet federal infection control rules, all of which are worth considering if someone wants the facts clear and upfront. The nurse turnover rate here stands at 58.3%, higher than the state average of 40%, although the registered nurse staffing level is 5.12 nurse hours per resident per day, which does go above California's state average of 4.5 hours. Folks get personal bedside TVs and dining options provided by a registered dietitian, featuring restaurant-style meals, selective menus, and special therapeutic diets, and there are staff who speak more than one language, so people from different backgrounds can feel comfortable. Residents get help with daily activities, can join in volunteer programs, and receive medical resources supported by a team that uses updated medical devices for diagnosis and care, which is helpful for those dealing with things like strokes, heart problems, diabetes, pain, or recovery from surgery, and there are services for speech therapy, rehabilitation, physical therapy, and pain management, so the care covers quite a lot. Detailed assessments, stabilization, tailored treatment programs, and transitional care are part of how they care for residents, and their emergency medical care covers a wide range of sudden needs. The atmosphere is aimed to be welcoming, and the team of doctors works together with a focus on the patient, using a teamwork approach for an inclusive treatment plan, and while the facility offers modern equipment and care for people with different medical challenges, it's important to weigh the facility's record of deficiencies and turnover alongside its care services.
