The center serves as a transition program for many patients, rehabilitating them for their return home with home health or outpatient services, or to an inpatient stay at Allied Services Rehab Hospital or Heinz Rehab Hospital, if appropriate. The skilled nursing care offered is designed for individuals who can no longer live safely and independently at home or at an assisted living facility.
For patients requiring an extended placement, the long-term care program at Allied Services Skilled Nursing Center offers specialized nursing care, diverse recreational activities, and supportive social services tailored to each patient's individual needs. The center's medical care is overseen by a physician in collaboration with an interdisciplinary team of licensed professionals. This patient-centered approach coordinates the patient's goals and lifestyle plan with their treatment, ensuring a holistic approach to care.
The center's short-term rehab track provides patients with intensive 24-hour nursing and therapy services, serving as a bridge between an acute care hospital stay and their return home.
