Licensed by the state of Wisconsin as a four bed adult family home, Northwest Transitions is contracted with St. Croix County Mental Health to provide crucial services as part of their community support program and emergency services. The ranch-style home features four single occupancy bedrooms, two kitchens, large dining and living room areas, a recreational room, and easy access to both the community and nature.
