The trained Direct Support Professionals at Prime Care Services offer high-quality services and support to individuals living in their own homes throughout the Twin Cities. The program currently serves individuals in Dakota, Hennepin, Ramsey, and Washington Counties who receive CADI, DD, AC, and Elderly Waiver Services. For those with higher care needs, Prime Care Services provides 24-hour supports in their two Community Customized Living Opportunities in St. Paul, helping residents maintain their independence in the community.
