New Hope Health Care II offers Customized Living Services for individuals living in a congregate setting, such as an Assisted Living facility. These services are designed to provide health-related and supportive care to residents, including up to 24 hours of supervision, assistance with activities of daily living, help with mental health cognitive or behavioral concerns, health-related assistance like medication management and delegated nursing tasks, home management tasks, non-medical transportation, and socialization.
The facility must be licensed as an assisted living facility, including those with dementia care, or as a Comprehensive Home Care provider delivering services in an affordable housing setting. Customized Living Services are available under waivers in the Community-Based Services Manual, offering a range of support to meet the individual needs of residents.
Operating from 8:00am to 5:00pm, seven days a week, New Hope Health Care II provides a safe and caring environment for individuals to receive the assistance they need. The staff is trained to meet the unique needs of each resident, ensuring they receive personalized care that promotes their overall well-being.
