Gordon Green is a residential treatment program that provides trauma-informed care and support services to adults living with mental illness complicated by co-occurring chronic health conditions. With a capacity of eight residents, the program offers a supportive, safe, and secure environment where individuals can receive psycho-social rehabilitation services and personal care. The goal of Gordon Green is to help residents with severe and persistent mental illness maintain their independence, enhance their quality of life, and access community opportunities.
Through an integrated health model of care, Gordon Green recognizes the unique needs of each resident and seeks creative ways to offer life-affirming and empowering experiences. The program is designed to address the complex needs of residents by utilizing an individualized, resident-centered approach managed 24-hours-a-day by staff trained in trauma informed care, mental health treatment, crisis intervention, and other best practices. Services provided include ongoing care coordination, meal preparation, assistance with daily activities, medical monitoring, medication administration, mental health treatment, transportation to recreational activities and appointments, organized community outings, and referrals to community resources.
Adults who have been diagnosed with a major mental illness and are living with co-occurring chronic health conditions are welcome at Gordon Green.
